Does Cigna Cover Ozempic in 2026?
Yes for type 2 diabetes, no for weight loss — and there is a quantity cap on it that Cigna will not negotiate. Most coverage pages stop at the first half of that sentence. The second half is the part that produces the pharmacy counter surprise.

Does Cigna cover Ozempic?
For type 2 diabetes, yes, subject to your plan's formulary and prior-authorization rules. Cigna's own policy states the two indications it recognises.
Note the second bullet carefully, because it is where people get their hopes up. Ozempic's cardiovascular indication is for reducing major adverse cardiovascular events in adults with type 2 diabetes and established cardiovascular disease. Wegovy's cardiovascular indication does not carry the diabetes requirement. If you have heart disease but not diabetes, the drug that fits that route is Wegovy, not Ozempic.
The cap with no override
Ozempic sits under a Drug Quantity Management programme, and the policy language on overrides is unusually absolute.
Most utilisation-management rules have an exceptions process. This one says it does not. If your prescriber plans to request an override on quantity, that is time spent on a door the policy states is closed — the productive conversation is about pen strength and fill timing instead.
One pen per 28 days, whatever your dose
The structure is one pen per 28 days at retail and three pens per 84 days through home delivery. Because the limit counts pens rather than milligrams, and each pen strength holds four weekly doses, the cap lines up with once-weekly dosing at every strength — which is the point. It becomes a problem in exactly two situations: a dose change mid-cycle that leaves you short, and a pharmacy filling early enough that the next refill falls outside the window.
The practical move is home delivery if you are stable on a dose. Three pens per 84 days is the same ratio with fewer chances to trip the clock.
Ozempic for weight loss on Cigna
Not covered, and no prior authorization changes it. Ozempic's approvals are diabetes approvals; weight management runs through Cigna's separate policy cnf_684, which lists Wegovy, Zepbound, Saxenda and Foundayo instead.
If semaglutide for weight management is what you actually want, the covered route on Cigna is Wegovy, and the bar is lower than most people expect: three months of behavioural modification rather than the six Aetna requires, and eleven qualifying comorbidities at BMI 27. What to check before the appointment
- Confirm the indication on the prescription. Type 2 diabetes puts Ozempic in play. Weight management does not, on any Cigna plan.
- Ask whether your plan applies prior authorization on top of the quantity programme. They are separate controls and both can apply.
- Switch to home delivery if your dose is stable. Three pens per 84 days reduces the number of refill clocks you can trip.
- Do not budget time for a quantity override. The policy says there are none.
