Does TRICARE Cover Ozempic in 2026?

Yes, for type 2 diabetes, with prior authorization — and because Ozempic sits on the diabetes list rather than the weight-loss list, the August 2025 coverage cut did not touch it. That distinction decides your answer, and most pages on this topic blur the two lists together.

We archived both official sources on July 26, 2026: the Express Scripts pharmacy FAQ and TRICARE's own FAQ. They do not say quite the same thing, and the difference matters if you are on a plan other than Prime or Select.

Does TRICARE cover Ozempic?

For type 2 diabetes, yes, with an approved prior authorization. Express Scripts, which runs the TRICARE pharmacy benefit, states it directly.

Read the two paragraphs as separate rules rather than one. The first covers Trulicity, Ozempic, Mounjaro and Victoza for type 2 diabetes. The second covers Wegovy, Zepbound, Contrave, Qsymia and phentermine for weight loss, and only for certain plans. Ozempic appears in the first list and not the second, which is why the 2025 change left it alone.

The August 2025 cut, and who it actually hit

As of August 31, 2025, weight-loss medications stopped being covered for beneficiaries outside the named plans. If you saw that headline and assumed your Ozempic prescription was at risk, it was not — provided it is prescribed for diabetes.

Here is where the two official sources diverge, and it is worth knowing which one to quote. The Express Scripts FAQ describes weight-loss coverage as being for "TRICARE Prime and TRICARE Select." TRICARE's own FAQ lists considerably more.

If you are on TRICARE Young Adult or Reserve Select and read only the Express Scripts wording, you would reasonably conclude you had been cut off. TRICARE's own list says otherwise. Quote the tricare.mil page when you call.

For the groups genuinely outside the list — direct care coverage and, in TRICARE's phrasing, "certain other groups" — the consequence is stated bluntly: you pay 100% of the cost of weight-loss drugs even with an approved prior authorization. An approval is not the same as a payment.

The 720-day rule almost nobody mentions

This is the most useful operational detail on the whole topic, and we have not seen it written anywhere else.

TRICARE's note reads: your provider must fill out a prior authorization form unless you have documentation for any other diabetes drug in your TRICARE pharmacy record in the last 720 days. Two years is a long window. If you have filled metformin or any other diabetes medication through TRICARE in that period, the paperwork step may simply not apply to you — worth checking before your prescriber spends a week on forms.

The same note flags that Trulicity carries a lower copayment than the others on the diabetes list. If cost rather than a specific molecule is what matters, that is a conversation to have with your prescriber.

Ozempic for weight loss on TRICARE

Not covered, and no prior authorization changes that. Ozempic's approval is for type 2 diabetes; weight management is a different indication with a different drug list. Asking for Ozempic on weight-loss grounds produces a denial that is about the list, not about you.

The realistic routes are narrow. If you hold one of the covered plans, the weight-management drugs TRICARE does list are Wegovy, Saxenda and Zepbound — same molecule in Wegovy's case, and our Wegovy coverage guides cover the criteria pattern. If you are outside those plans, you are paying full price regardless of what any authorization says, which makes cash-pay pricing the only comparison that matters.

What to ask, in what order

1. Establish which list you are on. Diabetes indication puts Ozempic in play; weight management does not, whatever your plan.
2. Check your pharmacy record for the 720-day window. Another diabetes drug in the last two years may remove the prior-authorization step entirely.
3. If weight loss is the goal, name your plan. Prime, Prime Remote, US Family Health Plan, Select, Young Adult, Reserve Select, Retired Reserve and CHCBP appear on TRICARE's list. Quote that page rather than accepting a "Prime and Select only" answer.
